The efficacy of allergen immunotherapy for the treatment of allergic rhinoconjunctivitis with or without seasonal bronchial asthma and anaphylaxis caused by the sting of the hymenoptera class of insects has been clearly demonstrated in numerous well-designed, placebo-controlled trials. Immunotherapy whether by subcutaneous injection of allergen extract or by oral/sublingual routes modifies peripheral and mucosal TH2 responses in favour of TH1 responses and augments IL-10 synthesis by TRegs both locally and by peripheral T cells. Recent researches into the cellular and molecular basis of allergic reactions have advanced our understanding of the mechanisms involved in allergic diseases. They have also helped the development of innovative approaches that are likely to further improve the control of allergic responses in the future. Novel approaches to immunotherapy that are currently being explored include the use of peptide-based allergen preparations, which do not bind IgE and therefore do not activate mast cells, but reduce both TH1 and TH2-cytokine synthesis, while increasing levels of IL-10. Alternative strategies include the use of adjuvants, such as nucleotide immunostimulatory sequences derived from bacteria CpG or monophosphoryl lipid A that potentiate TH1 responses. Blocking the effects of IgE using anti-IgE such as omalizumab, a recombinant humanized monoclonal antibody that selectively binds to IgE, has been shown to be a useful strategy in the treatment of allergic asthma and rhinitis. The combination of anti-IgE-monoclonal antibody omalizumab with allergen immunotherapy has proved beneficial for the treatment of allergic diseases, offering improved efficacy, limited adverse effects, and potential immune-modifying effects. This combination may also accelerate the rapidity by which immunotherapy induces TReg cells. If allergic diseases are due to a lack of allergen-specific TReg cells, then effective therapies should target the induction and the development of TReg cells producing cytokines such as IL-10.

آلرژی غذایی از شایعه ترین انواع حساسیت در کودکان می باشد (8% درکودکان زیر یکسال). در میان آلرژنهای غذایی پروتئین های شیر گاو از شایع ترین عوامل آلرژی زا در کودکان می باشد (50-45% موارد حساسیت غذایی)، تستهای تشخیصی آلرژی از قبیل تست پوستی یا اندازه گیری IgE اختصاصی، نیاز به آلرژنهایی با خلوص بالا دارند. هدف از انجام این مطالعه تخلیص 3 پروتئین اصلی و آلرژی زای شیر گاو یعنی کازیین، آلفالاکتالبومین و بتاکتوگلبولین به شکل عصاره آلرژنیک بوده است. ابتدا کازیین از شیر تازه گاو که چربی آن قبلاً جدا گردیده است، به روش اسیدی جدا شده و سپس آلفالاکتالبومین و بتالاکتوگلبولین از محلول رویی (سرم شیر) جدا گردید. آلفالاکتالبومین به وسیلة ژل فیلتراسیون از فراکشن رسوب داده شده سرم شیر توسط سولفات آمونیوم جدا گردید وبتالاکتوگلبولین نیز به وسیله کروماتوگرافی تعویض یونی از فراکشن رسوب داده شده سرم شیر توسط پلی اتیلن گلیکول جدا گردید. پروتئین های جدا شده توسط الکتروفورز پلی اکریل آمید و ایمونودیفیوژن دو گانه ( روش اخترلونی) ارزیابی گردیدند. نتایج نشان داد که کازیین به دست آمده در این مطالعه خالص تر از انواع تجاری موجود است به طوریکه خلوص آن حدود 95 درصد می باشد. آلفالاکتالبومین و بتالاکتوگلبولین به دست آمده نیز هر یک حاوی یک باند در الگوی الکتروفورز بوده و ناخالصی محسوسی در آنها مشاهده نشد. این نتایج بیانگر خلوص بالای محصولات به دست آمده است. این محصولات را می توان در راه اندازی تستهای اختصاصی مثل تست پوستی و الایزا به کار برد.

Since 20th century, when allergy was defined, an ongoing attempt for discovering the mechanisms underlying it and its treatment began. Defining allergens as well as cells such as regulatory T-cells and characterizing the antibodies involved in the pathogenesis (including blocking antibodies) have helped very much towards a better understanding of the immunologic process.However, Allergen specific immunotherapy (SIT), as a specific curative treatment for allergy also dates back to the beginning of the previous century and has progressed considerably during these years. SIT similar to natural immunomodulation, directs the immune response towards tolerance.New strategies in this field, such as using recombinant allergens, T- and B-cell-epitope-containing peptides, and DNA vaccination have shown promising results. Sublingual immunotherapy, although not yet FDA-approved, as an alternative strategy in SIT has demonstrated efficacy as well as safety.Furthermore, allergen extracts, their standardization and their modification have also been the focus of much research. Undoubtedly, specific immunotherapy is proven to be an efficacious method to treat allergy, so its cost-effectiveness should be estimated in developing countries in order to include it in the country's health priorities. Informing physicians about the new anti-vaccination movement is also crucial.

Background: In 2017, Turkey introduced Turkish Food Codex Regulation on labeling and providing food information to customers, which has been revised on a regular basis ever since. The purpose of this study was to survey food allergen information regarding foods and beverages sold in Turkey and the extent to which food allergens and precautionary allergen labeling (PAL) are emphasized in the products. Method: Thirteen food and drink categories were reviewed in five physical food stores and three online stores. A total of 1,010 foods and beverages were checked for allergens according to their labels. Results: 1,116 allergenic foods were identified in 59.7% of foods and beverages,. Gluten-containing cereals were clearly labelled in every food category, and milk received the highest number of declarations (30.55%), followed by gluten (27.42%), and tree nuts (9.67%). Snacks contained the most number of allergens (14.8%), followed by dairy products (13.8%), and baked products (12.4%). Conclusions: Accurate and understandable food allergy labeling is essential in avoiding food allergies. Food allergy labeling legislation especially The EU Regulation No. 1169/2011 establishes uniform rules for allergen labeling, which is one of the most stringent food regulations in the world. Current allergen labeling restrictions and policies need to be improved to reduce the risk of food hypersensitivity.

EDITOR: We read with great interest the article of Yadzir et al. 1 We would like to underline some adjunctive topics in favor of serum allergen-specific IgE (sIgE) assessment in AR patients management. Component resolved diagnostics (CRD) is particularly useful in AR management to correctly define real allergy, so to prescribe the causal allergen for AIT. We would like to remark another important consideration: polysensitization should not be an obstacle for AIT prescription. Introduction: Early life environmental factors, including exposure to allergens, are known to increase the risk of childhood allergy. However, it is not known if avoidance of allergens in early life can prevent allergic disease.Methods: A birth cohort of genetically at-risk infants was recruited in 1990 in a randomised, controlled study. Allergen avoidance measures were instituted, from birth, in the prophylactic (P) group (n=58). Infants were either breast fed with mother on a low allergen diet or given an extensively hydrolysed formula. Exposure to house dust mite was reduced by the use of an acaricide and mattress covers. Control (C) group (n=62) followed standard advice as normally given by the health visitors. Development of allergic diseases (asthma, atopic dermatitis, rhinitis, food allergy) and atopy (positive skin test) was assessed blindly at ages one, two, four and eight years in all 120 children (except 4 children missing skin test at 8 years).Results: Period prevalence of one or more allergic disease was significantly reduced in the prophylactic group at all ages. At one year; P: 13.8% vs. C: 38.7%, at two years; P: 25.9% vs. C: 50.0%, at four years; P: 32.8% vs. C: 53.2%, at eight years; P: 51.7% vs. C: 69.4%. There was a 2 to 5 fold reduction in the number of children with atopy at all follow-ups. When adjusted for confounding variables in a logistic regression model, the P group was confirmed to have significantly reduced risk at all ages. At one year; odds ratio (OR): 0.26, 95% confidence interval (CI): 0.08, 0.85; p=0.03, at two years; OR 0.17, CI 0.05, 0.53, p=0.002, at four years; OR 0.30, CI 0.11, 0.80, p=0.02, at eight years; OR 0.32, CI 0.12, 0.86, p=0.02.Conclusion: This study demonstrates that childhood allergic manifestations can be reduced by combined food and aero-allergen avoidance in infancy.

Background: Precautionary allergen labels (PAL) should be used to indicate the possibility of allergen presence in the food. This study aimed to determine the prevalence and types of precautionary labeling statements on different pre-packaged food products in retail stores in Belgrade, Serbia, as well as to assess consumers’,attitudes and behavior towards PAL statements. Methods: This was a descriptive study. The following characteristics of 1404 pre-packaged foods were analyzed: prevalence of PAL, listed food allergens on PAL, and the types of the advisory terminology. In the group of 275 participants (94 with food allergies, and 181 persons who purchasing food for a household member with food allergy) reading practice of PAL, purchasing practice based on PAL, and the opinion about PAL statements credibility were evaluated. Results: Overall, 33. 9% of products had precautionary statements for one or more allergens. “, Tree nuts”,were the most common allergens listed in the PAL. The most common type of PAL was “, May contain traces of x [allergen]”,(52. 7%). The PAL was always read by half of the participants. Less than half (43. 3%) of the participants incorrectly believed that PAL is regulated by national law. A quarter of participants thought that the PAL statements are trustworthy. Conclusion: PAL statements frequently are not user-friendly and are not providing sufficient protection for food allergic patients. To gain buyers’,confidence, protect health and provide security, the necessity for the strategies that would regulate PAL by the law exists.
